I do hereby agree to hold GSLAHE , First Evangelical Free Church, Verity Institute and/or their volunteers harmless from any and all liability, actions, causes of actions, claims, expenses, and damages on account of injury to my child or property. In the event that my child becomes injured or ill during any game or activity, I authorize GSLAHE and their representatives to secure first aid and/or services of a physician or hospital and agree to assume all financial obligations incurred therewith.

In Internet Explorer - please click on "View" in your browser tool bar, then slide your cursor down to "Text Size", on the menu that pops up while you hold your cursor over "Text Size" please choose "smaller" or smallest".

Then click on "printer friendly" below these instructions. When the new screen pops open, please click on "File" in your browser tool bar at the top of your page, then click on "Print Preview". If the entire form is not showing on one page in the "print preview" screen, then click on the drop down menu that is at the center of the top of the screen that says "shrink to fit" or a percentage such as "90%". In the "Scale" drop down box please choose 70% and make sure that the entire form is showing on one page, including the "total" line. If part of the printing instructions appear on the page, do not worry. Next click "Print" on the left side of your screen.
