Before printing the form
please click on “View" in your browser tool bar at the top of your screen.
Then scroll down to "Text Size" and click on "medium".
This will allow you to have plenty of room to fill in the form once it is printed.
Next you click the "printer friendly" button.
You may then click on "File" in the upper left
hand of your browser tool bar.
Next scroll down to and click on "print preview" to
make sure the form will fit on the number of pages you want to print out.
At the top of the print preview
screen you will see a drop down menu that allows you to shrink the size of the page you will be printing. You can either
choose "shrink to fit" or a percentage, such as 90%.

plenty of room to fill in the form once it is printed.
Next click on "printer
friendly" at the bottom of the screen below the form you wish to print.
Then click on "file"
in the upper left corner of your browser window and then scroll down and click "print preview" to
make sure the form will fit on the number of pages you wish.
